Skilled Nursing services include a variety of tasks, including post-acute care, observation and assessment, medication administration and management, wound care to include wound vacs, pain management, as well as educating caregivers and patients regarding disease management of specific conditions such as Congestive Heart Failure, COPD, and Diabetes. Other nursing services provided are ostomy care, IV therapy, injections (and teaching you and/or your caregiver to self-inject), tube feedings, catheter care and catheter changes, tracheostomy care, cardiac rehabilitation, laboratory and x-ray imaging, observation and assessment of your condition along with medication teaching and weekly set ups.

Physical Therapy Services include techniques to improve endurance and mobility and are oftentimes critical to post surgery recovery gait training. They also provide education regarding exercises to regain movement and range of motion, strength and balance training, pain management, vestibular treatment, as well as assisting patients/clients with mobility and ambulation, with consistency to Plan of Care. Examples may include, transfers, assistance with walker use and other assistive devices.

Speech-Language Pathology services include exercises to regain and strengthen speech and language skills, cognitive training and recall, dysphagia and aspiration precautions and treatment, assistance in regaining abilities following a stroke and also helping patients who have difficulties swallowing as a result of a medical event, as well as energy conservation techniques.

Occupational Therapy helps you regain the ability to do usual daily activities by yourself, such as eating, dressing, bathing, cooking, low vision training, energy conservation, dexterity movements, writing, etc.

Home Health Aide includes a home health aide who provides personal care services, including help with bathing, toileting, and dressing.

Medical Social Services include assistance with social and emotional concerns you have related to your illness, community resources for psychosocial needs, counseling services, and assistance finding necessary medical and safety equipment.
